To cancel your membership, please download the fillable cancelation form PDF, save it to your computer, complete all fields and then upload the form in the portal below.
Cancelation forms must be returned a calendar month in advance. Once received you will still have one more payment taken from your account and your membership will remain active until the last day of the following month. For example, a cancelation request submitted in October, will have a debit on the fifth (5th) business day in November and will expire on November 30.
If you are canceling a 12-month self-renewing contract within the first twelve months of membership, a cancelation fee equal to the remaining monthly dues of the membership term will be charged.
Joiner’s fees and/or prepaid dues are not refundable.
By canceling your membership, you forfeit any unapplied insurance reimbursement credits.
